Mobile (CoR) Forms

The Form Reports module allows you to add custom forms to the mobile application to collect data from users. Forms can be filled out manually or triggered to appear automatically based on events.

Complete a form

The process for filling out forms is the same whether they are triggered automatically or accessed manually.

Access the forms list

  1. Tap the Menu icon (☰) on the mobile app.
  2. Tap Do A Report. A list of available reports displays.

Select and complete a form

  1. Tap the applicable report from the list.
  2. Answer each form question.
  3. Tap Next to proceed to the next question.
  4. Tap Back to return to a previous question if needed.

Submit the form

  1. Tap the Finish button on the last screen. Your answers are saved automatically.

Edit form questions

  1. Go to Reports > Configuration and Setup > Mobile Form Template on the web portal.
  2. Edit the questions as required.
